2026/27 Tryout Forms + Links
Any 6-11th graders attending an Open Gym should fill out the Open Gym Waiver.
Send TWO core subject teachers the link.
This Google Form is only accepting responses from ISD 728 accounts, and teachers who are sent the link will have no problem filling out the form. Parents do not need to request access to the form.
Please do not email Coach to ask the status of teacher recommendations. You will be notified the week of tryouts if you are missing your recommendations.
